Please understand that with HMO insurances it is required to have a specialty referral form from your plan filled out by your dentist prior to the appointment.

We will submit all claims with all other insurances as long as the benefits can be assigned to our office.  The patient will be responsible for any balance that it is not covered.  Please check with your insurance carrier if you have any questions regarding your plan.   Yearly maximums and deductibles may apply.

As a courtesy to our patients, we also accept care credit, which offers up to 1-year payment plans at no interest.
